I the parent/guardian of the above named, hereby give my approval to his/her participation in all soccer activities during the current season & assume all risks & hazards incidental to the conduct of the activities and transportation to/from activities. I further release, absolve, indemnify and hold harmless the "Chornomorska Sitch", and the organizers & supervisors. In case of injury to my child, I hereby waive all claims to the organizers, sponsors, or any of the supervisors appointed by them.
